Refund Request

1. All requests are subject to approval and must be submitted through this form. The request must include: the name of the team, division, contact person, organization/person refund is to be made out to and mailing address.

2. Requests received prior to JANUARY 20, 2024, will receive a full refund minus a $50.00 administration fee.
3. Requests received after JANUARY 20, 2024 will receive a full refund minus a $50.00 administration fee only if a replacement team can be found or they are entered into OMHA playoffs.

4. NO REFUNDS will be provided after FEBRUARY 24th, 2024.

Refunds will be provided in the form of cheque. Please allow four to six weeks to process the request.
